ALVIS HOUSE BENEFITS SUMMARY
HOLIDAYS
Alvis House observes nine (9) holidays. Regular hourly
full time or part time employees who work on a holiday will receive two
times their regular pay rate for the hours they work. If an employee
does not work on the holiday, s/he will receive eight (8) hours holiday
pay at their regular pay rate. Part time employees' holiday pay is
pro-rated to the average hours worked in a pay period.
VACATION
After six (6) months continuous service, a regular,
nonexempt employee earns one week (5 days) of vacation. After one (1)
year continuous service, s/he earns one more week of vacation. From two
to four years of service, s/he will earn two weeks of vacation after
each year of continuous service. After five (5) years of service, s/he
will earn three weeks of vacation. After ten (10) years of service, s/he
earns four weeks of vacation.
SICK LEAVE
All full time employees earn sick leave at the rate of
6.67 hours per calendar month. Part time employees' sick leave is
pro-rated according to the hours usually worked. Hours not used during
the calendar year are carried over to the following year. You may
accumulate an unlimited amount of sick leave, but may only use up to 480
hours per sick leave per occurrence.
UNUSED SICK LEAVE BONUS BONUS
A bonus of 50% of the earned, but unused sick leave will
be paid in December of each year. The bonus is calculated based on an
employee’s total sick leave awarded from the previous December 1 through
November 30 of the current year, minus any sick time used during that
period and then divided by two. This gives employees the opportunity to
earn an annual bonus with a maximum equivalent of one week’s pay. Bonus
hours will not be deducted from the total sick leave earned during the
12-month period.
EMPLOYEE ASSISTANCE PLAN
Confidential assistance is available for employees and
their families through Life Phases, a comprehensive health information,
education and support service. Life Phases can provide help with: minor
medical issues, substance abuse, family and relationship problems, grief
and loss, depression, smoking cessation, personal legal and financial
issues and workplace concerns. Alvis House provides this program at no
cost to staff.
SHIFT DIFFERENTIAL
Employees who work at least seven (7) hours between the
hours of 10:00 p.m. and 8:00 a.m. receive an additional $0.25 per hour
for all hours worked during that period.
LONG TERM DISABILITY
Employees who work at least 30 hours per week are
enrolled in the agency’s long-term disability protection plan (through
UNUM) after six months of employment. The plan provides for the payment
of monthly benefits if an employee becomes totally or partially disabled
for 90 days or more and is unable to work on a full-time basis. The plan
provides up to 60 percent of an employee’s basic monthly salary, up to a
maximum of $3,000.00 monthly. Alvis House pays 100% of the premium.
SHORT TERM DISABILITY
Employees who work 30 hours or more per week are
enrolled in the agency’s short term disability protection plan (through
UNUM) upon completion of one year of service. Alvis House pays 100% of
the premium. The plan provides for the payment of weekly benefits if an
employee becomes totally or partially disabled and is unable to work on
a full-time basis. The plan provides up to 60 percent of an employee’s
weekly salary, up to a maximum of $500.00 weekly. Benefits may begin on
the 15th day of a disabling injury or illness and may continue for a
maximum of 11 weeks.
LIFE INSURANCE
Employees who work at least 30 hours per week are
enrolled in the agency’s life insurance plan (through UNUM) after six
months of employment. Alvis House pays 100% of the premium. The benefit
amount is two (2) times the annual salary.
PENSION PLAN
Alvis House provides a retirement program for all
qualifying employees through the United Way Master Pension Plan.
Employees of Alvis House are enrolled in this defined benefit pension
plan after completing 1,000 hours of service in one calendar year; and
are fully vested after completing five (5) consecutive years of service
in which they work at least 1,000 hours of service. Alvis House pays
100% of the cost of the pension plan.
OPTIONAL BENEFITS (Health Insurance,etc.)
Alvis House provides employees with a number of benefit
options to help provide for the health and welfare of themselves and
their families. Alvis House pays 70% of the cost of the monthly premium
for full-time employees who elect to participate in the agency’s group
health insurance plan. Other benefits currently available to employees
are dental insurance, optional life insurance, hospital expense
protection, cancer expense protection, and personal short and long
term disability insurance. Part time employees who regularly work at
least 20 hours per week will receive a pro-rated amount that they may
apply toward plan premiums. Insurance coverage is effective the
first of the month following 30 days of employment
